Mechanism
  • Action
    • prevent viral uncoating
      • by binding to protein M2
      • this protein normally acts as an ion channel that allows acidification of the viral core to activate viral RNA transcriptase

  • Resistance
    • mutated M2 protein confers resistance
    • 90% of all influenza A strains are resistant!

Uses
  • Influenza A
    • NOT influenza B
    • used for both prophylaxis and treatment
  • Parkinson's disease
Adverse effects
  • CNS toxicity
    • dizziness
    • ataxia
    • slurred speech
  • GI upset
  • Rimantadine has similar efficacy, fewer side effects and does not require dosage adjustments in renal failure
Pharmacology
  • Amandatine crosses the blood-brain barrier
  • Rimantadine does NOT cross the blood-brain barrier
