  • Impaired sperm production or delivery
    • production problems may be of primary or secondary causes
      • primary infertility
        • Leydig and/or seminiferious tubule dysfunction
        • etiologies include
          • varicocele (increased temperature of sperm)
          • Klinefelter syndrome
          • orchitis
      • secondary infertility
        • pituitary/hypothalamus dysfunction
    • delivery problems can involve
      • disorders of ejaculatory path
        • e.g. obstruction of vas deferens
      • dysfunction of accessory sex organs
        • results in inhospitable fluid for sperm deposition in vagina
      • hormone as well as sperm levels are normal
Evaluation
  • Semen analysis
    • gold standard
    • measure semen volume, sperm count, morphology, and motility
  • Serology
    • measure LH, FSH, testosterone, and prolactin
