Introduction
  • A conditioned involuntary contraction (spasm) of the lower vaginal muscles
    • results from a woman's unconscious desire to prevent penetration
  • Vaginismus is a learned response
    • often due to dyspareunia
    • even after the cause of dyspareunia has been removed, memory of the pain can perpetuate vaginismus
  • Other causes include fear of
    • pregnancy
    • being controlled by a man
    • being hurt during coitus
Evaluation
  • Involuntary vaginal spasm during pelvic examination confirms the diagnosis.
Treatment
  • Painful physical disorders should be corrected
  • If vaginismus persists, techniques to relax the muscle spasm are effective
    • gradual dilation
    • counseling
